(3) MACPAC tiers: extensive and limited make up the 40 adult-coverage jurisdictions; emergency and none are the remainder, not a second 51-count. (4) Uncovered-basket exposure is uncovered codes priced at that state's private estimates. Always verify current fees and benefits before relying on a single cell.
